Fish River Rural Health’s Registered Nurses coordinate care with patients and their primary care providers to manage chronic conditions, such as diabetes, high blood pressure, heart disease, asthma and other illnesses that require ongoing clinical care.  In addition to shared decisions with patients and providers, Registered Nurse Care Managers provide clinical education and guidance related to multiple disease processes, medication management, nutrition, physical activity and smoking cessation.
