In May of 2002, FRRH was selected by the Bureau of Primary Health Care to participate in the HDC. Phase I kicked off at the first learning session on July 11, 2002 and came to a close at National Congress in September 2003. Since inception, noteworthy changes have occurred in redesigning our delivery system and incorporating the Chronic Care Model in order to provide improved care for patients with diabetes. FRRH first selected all diabetic patients in Paul Pelletier, M.D., Medical Director’s panel at the Eagle Lake site. The initial size of the population of focus was approximately 100 patients and new patients added as they came in the practice and/or initially diagnosed. As of April 2008, there are a total of 185 diabetic patients in the registry, which includes all patients within both FRRH practices. FRRH has expanded to Cardiovascular at both FRRH sites with 453 patients in the registry. |
