Family Medicine

Fish River Rural Health provides a full spectrum of comprehensive primary health care services by board-certified physicians, physician assistants and nurse practitioners.  The patient-centered approach strengthens the provider-patient relationship by replacing episodic care with coordinated care focusing on preventive health. Patients develop a lifetime relationship with the primary care provider who leads a team that takes collective responsibility for patient care, providing for the patient’s health care needs and arranging for integrated care with other qualified providers. This approach results in more personalized, coordinated, effective and efficient care.

 

Preventive Care

Preventive care and diagnostic medical care both play an important part in keeping a patient healthy.  Preventive care focuses on assessing the patient’s current health status when they are symptom-free.  Examples include immunizations, lab tests, cancer screenings and other services intended to prevent illness or detect problems before the patient notices any symptoms.  Examples of preventive medical care include:

  • Annual Physical

  • Medicare Wellness Visit

  • School & Sports Physicals

  • Well Child Check

  • Prenatal and Perinatal Care

  • Post-Partum Care

  • Immunizations

  • Preventive Screenings: breast cancer, cervical cancer, and colorectal cancer screenings.

  • Health Education

 

Chronic Care

Fish River Rural Health provides an organized team approach to chronic disease care.  The team consists of patient, provider, registered nurses, medical assistants, behavioral health professionals, specialists, and other healthcare professionals.
Examples of chronic care management include:

  • Diabetes

  • Hypertension

  • Asthma

  • Heart Disease

  • Depression

 

Acute Care

Acute care is the diagnosis and treatment of short-term, non-life-threatening injuries and illnesses.  Fish River Rural Health offers high quality medical care for acute care situations.
Examples of Acute Care include:

  • Allergies

  • Asthma/Breathing Treatments

  • Dehydration

  • Earaches

  • Fever/Cold/Flu

  • Headache/Migraine

  • Insect Stings

  • Sore Throat

  • Sprains/Dislocations/Fractures

  • Stomach Pain

  • Wound Care or Lacerations

 

Commercial Motor Vehicle Exams

  • At all Fish River Rural Health site locations, the Agency has certified Commercial Motor Vehicle Medical Examiners.
